Table Description:  The table contains individual death records that do not fall into any specific collection (such as the Illinois Pre-1916 Death Record collection). The amount of information contained in these records varies from minimal name and death date to extensive information including parents and cause of death. In all cases, the state and county are indicated for each record.



Name of Table:  Illinois Statewide Death Index 1916 -1950 (View Records!)
Table Description:  This database provides listings of death certificates filed with the Illinois Department of Public Health between 1916 and 1950. As a result of 1989 legislation, that agency makes available for public inspection at the Illinois State Archives only copies of death certificates produced 50 years ago or longer.

Certificates show county and city in which death took place; certificate number; address where death took place; sometimes the voting ward of that address; the deceased's full name, sex, marital status, birth date, age at death, occupation, employer, and city and state of birthplace; father's name and birthplace; mother's maiden name and birthplace; name of the informant providing the above information; the filing date; and the name of the registrar. Also included are the date of death; indication if an inquest was held; sometimes the duration of the causal condition and the type of secondary contributory cause; the coroner's signature, address, and telephone number; the date of the coroner's signature; sometimes the length of residence if at an institution and the place where the contributory disease was contracted if not at the place of death; the burial place and date; and the undertaker's name and address.

Name of Table:  Illinois Statewide Death Index pre-1916 (View Records!)
Table Description:  The pre-1916 Illinois death index is an ongoing project coordinated by the Illinois State Archives and supported by the Illinois Secretary of State's Department of Information Technology. Volunteers from around the world are currently entering data into this death index from their home computers through the Illinois State Archives Web site.

The sources for this index include original county clerks' death records, such as death registers and licenses. For each death, the index includes the name of the decedent, the date of the death, the name of the county where the death occurred, the place of death within the county, when possible; the age and sex of the decedent and a citation to the original record'volume and page number for death registers or certificate number for death certificates. Death certificates show the name, age, sex, marital status, and race of the deceased; the places of birth, death and burial; the dates of death and burial; the cause of death; the date filed; and the signature of the physician and the registrar.

Death record or register show the name, race, marital status, age, sex, and occupation of the deceased; the date, place, and the primary cause of death;
contributing causes and duration; the place and date of burial; the name and address of the undertaker; and the name and address of the physician.

Name of Table:  Michigan Death Index 1867 - 1889 (View Records!)
Table Description:  This index covers all of the Williams' found in the Michigan State Death Index from 1867 thru 1889. The death records for these individuals can been viewed at their website at http://www.mdch.state.mi.us/gendisx/search.htm and contain the following pieces of information: Date of death, Ledger Page, Record Number, Place of death, County of Death, Sex, Race, Marital Status, Age, Cause of Death, Birthplace, Occupation, Father's Name, Father's Residence, Mother's Name, Mother's Residence and Date of record.

Names are in the form last name, first, middle. The notation Not Recorded or NR above indicates that the ledger page was left blank in that spot.



Name of Table:  Williams Death Records in Missoui - 1883 to 1955 (View Records!)
Table Description:  This table contains an index of death certificate for Williams who died in Missouri between 1883 and 1955. The data contains a combination of Name, County of Death, Date of Death, and in some cases, city of death, age, race, sex and certificate number.

Name of Table:  Orleans Parish, Louisiana Death Index (1864-1915) (View Records!)
Table Description:  The following information was imported by Adrian Williams from the Orleans Parish Rootsweb website with permission from Colleen Fitzpatrick. The records were extracted from the Death Rolls of Orleans Parish by New Orleans Volunteer Association (NOVA), which consists of over 200 volunteers. To receive a copy of any of the certificates in any of the Birth, Death or Marriage Indexes, Please send $5.00 with the NAME, volume & PAGE # to the LA Archives at the address below. During early years when death certificates were not legally required, some deaths may not have
been registered. If your name should be on this list, but it is not, the State Archives does
not have a record of it.
